了解《预先医疗指示》（Advance Health Care Directives）及其填写方式，学习如何指定代理人并做出临终决策。了解何为 POLST 表格，以及如何通过该表格明确您愿意接受的紧急医疗护理类型。
获取其他规划工具的推荐，例如遗嘱和财务持久授权书（Durable Power of Attorney for Finance）。如往常一样，欢迎您带着问题和见解参加。
本次活动由法律援助长者服务组织（Legal Assistance for Seniors）提供支持。讲座将由Edmund Lam先生主讲，他将使用普通话进行讲解，并可用粤语沟通。

Make a plan now for the medical care you want to receive if you become seriously ill or unable to communicate your wishes.
Learn about Advance Health Care Directives and how to complete one. Find out how to appoint an agent and make end-of-life decisions. Discover what a POLST form is and how it can specify the type of emergency medical care you are willing to receive.
Receive referrals for other planning tools such as wills and durable powers of attorney for finance. As always, you are invited to bring your questions and your insights.
